[ ] Confirm the party name and address match the contract. [ ] Confirm the endorsement matches the relationship: owner, lessee, contractor, landlord, public agency, lender, or other party. [ ] Confirm whether ongoing operations, completed operations, or both are required. [ ] Confirm whether the contract requests CG 20 10, CG 20 37, a specific edition date, or equivalent wording. [ ] Confirm whether primary and noncontributory wording is required. [ ] Confirm whether waiver of subrogation is separately required. [ ] Confirm whether umbrella or excess additional insured status is required, if the contract asks for it. [ ] Confirm the endorsement does not add limits; all insureds share the policy limits. [ ] Attach the endorsement or carrier-issued evidence showing the exact party, project, and wording.

GC request example

Use this format when the contract request comes from a general contractor. Additional insured party: General contractor and project owner named in the contract. Project/location: Project name and job address listed in the subcontract. Forms requested: CG 20 10 for ongoing operations and CG 20 37 for completed operations, or equivalent carrier forms if accepted by the contract. Primary wording: Required when the contract says coverage must be primary and noncontributory. Waiver: Required only if the contract separately requests waiver of subrogation. Limits/notes: Enter the contract limits, completed-operations period, and any exact wording the carrier must review.
