Project details

Business: ________________ General contractor or requester: ________________ Project: ________________ Project state: ________________ Required limits from contract: ________________

Use this checklist with the contract insurance exhibit. Remove any item the contract does not require before you send the request.

Contract wording checks

  • Confirm the named insured on the policy matches the business listed in the contract.
  • Confirm the required general liability limits match the contract: ________________.
  • Additional insured requirement: ________________. Ask whether the contract requires ongoing operations, completed operations, or both.
  • Completed operations requirement: ________________. Roofing claims can be reported after work is finished, so confirm the endorsement wording if the contract asks for it.
  • Primary and noncontributory requirement: ________________. Confirm whether the policy includes this wording before the certificate is issued.
  • Waiver of subrogation: review the contract and mark this item if the contract requires the insurer to waive recovery rights against the requester.
  • Certificate holder details match the contract, including legal name, mailing address, project name, and any loan or owner reference.
  • The certificate shows current policy dates and the limits required by the contract.
  • Any required endorsement copies are attached when the requester asks for policy wording, not only a certificate.

Questions for review

Send these questions with the contract insurance exhibit when a requirement is unclear.

  • Does the contract require additional insured status for the general contractor, owner, developer, architect, property manager, or another party?
  • Does the contract require completed operations additional insured wording after the roofing work is finished?
  • Does the contract require primary and noncontributory wording?
  • Does the contract require a waiver of subrogation on general liability, workers compensation, commercial auto, or more than one policy?
  • Does the contract require umbrella or excess liability limits above the primary policy limits?
  • Does the requester need endorsement copies, or is a certificate of insurance enough for its review?

Review notes

Notes from carrier or insurance contact:

RequirementConfirmed wording or next action

Additional insured | Completed operations | Primary and noncontributory | Waiver of subrogation | Required limits | Certificate holder | Endorsement copies |

Next steps

  • Compare the finished checklist with the contract insurance exhibit before requesting the certificate.
  • Ask your carrier or insurance contact to confirm endorsement wording, not only certificate details.
  • Remove checklist items that the contract does not require before sending the request.
  • Keep the completed checklist with the contract and certificate for the project file.
