Job and contract details

Business: ________________ Contract type: ________________ Certificate holder: ________________ Project or job: ________________

Use this checklist when reviewing your masonry policy before requesting a certificate of insurance. Walk each line through with your carrier and note the policy number, limits, and form details where prompted.

Policy number (general liability): ______________ Policy effective dates: ______________ to ______________ Named insured on the policy matches the business name above: yes / no

General liability endorsements to verify

Confirm the general liability policy carries the endorsements your contract requires. The contract clause that triggers each endorsement should be quoted in your file.

  • Additional insured for ongoing operations. Note the endorsement document identifier and edition date from the carrier: ______________
  • Additional insured for completed operations. Masonry work is sometimes flagged as damaged after the job is finished, so the ongoing operations endorsement alone may leave later claims outside the additional insured grant. Note the endorsement document identifier and edition date: ______________
  • Primary and noncontributory wording, if the contract asks the policy to respond first and the certificate holder's insurance to apply only above your limits
  • Waiver of subrogation in favor of the certificate holder, if the contract asks for it
  • Per project aggregate, if the contract or job requires a separate aggregate for this project

For each endorsement, ask the carrier to confirm the exact endorsement document identifier and edition date, not only that the endorsement is on the policy. Older and newer additional insured endorsements use different trigger wording and one may be narrower than the other.

Contract clause that triggers each endorsement: ______________ Carrier confirmation date: ______________

Limits to verify match the contract

Compare each policy limit to the minimum the contract sets before sending the certificate.

General liability per occurrence: ______________ General liability general aggregate: ______________ Products and completed operations aggregate: ______________ Commercial auto combined single limit: ______________ Workers compensation employer's liability: ______________ Umbrella or excess liability limit, if the contract requires excess coverage: ______________ Tools, equipment, or inland marine limit, if the contract requires evidence: ______________

A certificate that shows lower limits than the contract requires may be rejected by the certificate holder.

Documents the requester may ask for

Some general contractors, owners, and public agencies ask for more than the certificate of insurance itself. Confirm which of these the contract or requester wants before sending.

  • Copy of the additional insured endorsement form, not just the certificate line item
  • Copy of the waiver of subrogation endorsement
  • Copy of the primary and noncontributory endorsement
  • Declarations page showing named insured, policy period, and limits
  • Excess or umbrella policy binder or declarations, if a higher limit is required
  • Proof of workers compensation coverage on the state-required form
  • Description of operations text the certificate holder asked you to include

Requester contact for these documents: ______________

Notes and follow-up

Items the carrier still needs to add or change: ______________

Endorsement effective date once added: ______________

Certificate sent on: ______________ Certificate sent to: ______________

Renewal reminder: confirm the same endorsements stay on the policy at the next renewal so the certificate keeps showing the wording ________________ requires. Save this checklist with the signed contract and the issued certificate in the project file.

Next steps

  • Read the insurance section of the contract side by side with this checklist before contacting your carrier.
  • Ask the carrier to confirm form numbers and edition years, not only that an endorsement is included.
  • Keep this checklist with the contract and the issued certificate in the project file for the job.
