Standard COI fields

Check these items before sending the certificate.

  • Named insured matches the legal business name used for the job.
  • Certificate holder name and address match the customer, GC, or property manager request.
  • Policy numbers are shown for each listed policy.
  • Effective and expiration dates cover the expected work period.
  • General liability is listed if the request asks for proof of liability coverage.
  • Commercial auto is listed if a business vehicle is part of the request.
  • Workers compensation is listed if employees or the contract request require it.
  • Description of operations identifies the pressure washing job or site without adding unsupported coverage wording.

Limits and coverage check

Use the customer request or contract as the source for limit checks.

  • Each requested limit is shown on the COI or addressed before sending.
  • No fixed limit is assumed when the request does not state one.
  • Tools, equipment, or property coverage is not presented as general liability coverage.
  • Pollution, wastewater, runoff, old paint, or chemical exposure is escalated for policy review if the job involves those hazards.
  • Surface damage concerns, such as siding, glass, paint, or concrete discoloration, are noted for review if the job scope raises them.

Endorsement wording

Confirm endorsement wording instead of relying only on a certificate note.

  • Additional insured request is reviewed against the policy endorsement.
  • Ongoing operations additional insured status is checked if requested.
  • Completed operations additional insured status is checked if requested.
  • Primary and noncontributory wording is confirmed if requested.
  • Waiver of subrogation is confirmed if requested.
  • The form number and edition date are recorded or reviewed when the customer requests specific wording.
  • The certificate does not promise coverage that the policy or endorsement does not provide.

Before sending

Final send review.

  • The contract, request email, or portal instructions have been checked for exact certificate wording.
  • The certificate holder and endorsement request are consistent with the contract party.
  • Any mismatch between requested wording and policy forms has been escalated to the agent or carrier.
  • A copy of the sent COI and request is saved with the job file.
  • If work involves storm drains, rinse water, chemicals, or old paint, wastewater and pollution questions are documented before work begins.
